Most sports require a copy of the athlete’s birth certificate.
To facilitate the registration process, please check out this list:
Birth Certificate Information on File (5/15/08): Certificates on File.xls
If your child’s name is in it, we already have the birth
certificate information on file (you do not need to submit a copy of the birth
certificate with the application).
